1. Risk. I understand that soccer has a risk of injury. I accept that risk for my child.
2. Rules. My child and I follow the rules and the code of conduct of Ontario Soccer, of the district and of the club.
3. Medical care. I allow the club and Ontario Soccer to give first aid to my child and to call emergency services when needed.
4. Information. Ontario Soccer and Canada Soccer keep the registration record of my child in the national registry. They share it with the club and with the district.
5. Photos. The photo consent above applies to the club only. Ontario Soccer asks its own consent for its events.
